Berkshire College Dental Clinic Patient Treatment Consent

Our goal is to provide you with quality dental products at the best prices.  We do our training in regular dental offices so you will be receiving dental care using the same technologies and products that a regular dental office uses.  Your care will be provided by dentists and dental hygienists licensed by the State of Washington.  We train dental assistants and dental administrative staff. 

You should expect to spend a great deal of time waiting before, during, and after your treatments.  To keep you entertained while you wait, we offer the following:

  • Hot and cold beverages (coffee, cappuccino, hot chocolate, pop, juice, water)
  • Snacks (cookies, fruit packs, chips, rice krispie treats)
  • DVD movies in waiting room
  • Video games
  • Computer with internet access and laser printer
  • Free long distance phone calls anywhere in US
  • DVD movies in dental chair

 

In order to provide you with the most savings in your dental care, we would like to maximize our efficiency and work as follows:

  1. When contacting us, email is preferred and will be responded to faster than phone calls.
  2. Prior to coming to our clinic, we ask that you pay for any potential treatment upfront.  If you do not complete treatment here, your money will be refunded within one month of a request for a refund.
  3. We do not schedule appointments for most patients.  After paying, you sign up to be on our email list.  In the mornings, we send out email announcements of when we are available to see patients that day.  The earlier you come to our clinic that day, the greater chance you will be seen.  If you do not have email, you will need to find someone who does and who can contact you immediately after receiving our email announcements.  We cannot guarantee that you will be seen the day that you come because sometimes we get too many patients.  Other times we may choose to do certain procedures before others.
  4. If we do schedule an appointment for you for select procedures, you must be at the clinic at least 30 minutes before your appointment time.  If you are not here 30 minutes in advance for any reason, you will be charged a fee of $100-$200. This fee is not applicable if you do not schedule an appointment. 
  5. When you are in the dental chair, we spend our time doing clinical treatment only and cannot do any consultations or engage in any other discussions.  Clinical time is very expensive because of the cost of dental equipment, facilities, and clinical staff.  We offer low prices by maximizing the use of our facilities and clinicians.  So when you are in the dental chair, we need to focus on doing dental treatment only and nothing else.  If you have any questions, whether they be clinical or financial, please ask them well in advance of your visit to our clinic.
  6. There is an additional charge of $100 per consultation if you would like to discuss your treatment in person with our clinical staff of dentists and dental hygienists.  You can avoid that fee by sending your questions to our receptionist and she can in term find out the answers from the clinical staff and relay them back to you.  This can be done via either the telephone or email.  Email questions are encouraged.
  7. When you are at our clinic, we appreciate it if you could minimize any distractions to our staff by not engaging in any conversations with them.  That way, our staff can focus on working more efficiently and reducing your costs.
  8. We do not require you to have an exam or x-rays done at our clinic if you have had them within one year of coming here.  Just get a copy of your treatment plan and x-rays from your previous dental office.  X-rays must be clear and of good quality to be usable.
  9. We encourage you to get an exam and x-rays at a regular dental office first so that you can decide on what treatment you want to have done prior to coming here.  We would like to focus on doing the clinical treatment only.
  10. We do not provide any treatment for dental emergencies.
  11. There may be a wait of several weeks between visits to our clinic.  Should you have any problems following dental treatment at our clinic and require urgent attention, you will need to go to a regular dental office and pay regular fees. 
  12. Should any problems arise from your treatment here, your only legal recourse is to seek a refund of any fees that you paid and nothing else.

To become a patient at our office, you must agree to all the above terms.
